CCRCs offer seniors a fulfilling lifestyle with the assurance that extra assistance and health care will be provided as needed. Concordia provides opportunities for a wide range of spiritual, cultural, educational and recreational pursuits. Opportunities include a multi-purpose room for community gatherings, living room, coffee nook, exercise room, walking paths, computer station, card and game rooms, gardening area with raised garden beds, library, chapel, and beauty shop. We also provide regularly scheduled transportation to a wide variety of popular area activities.
A continuing care retirement community, also known as a life plan community, provides a quality, residential living environment in which senior adults live full, independent lives as actively as they wish, with the assurance of on-site assisted living, rehabilitation, and nursing care if needed. This makes them differ from monthly rental communities – by offering guaranteed health care should the need arise.
Continuing care retirement communities provide private residences in a community setting. Residents can enjoy common areas for dining and other activities, as well as a wide range of convenient services and amenities.
At Concordia Life Care Community, spacious residential apartments and patio homes are offered in a campus-style setting offering stimulating activities, dining choices and social interaction for independent living, assisted living and memory care. Nursing and Rehabilitation are available on-site in a Medicare certified nursing center. Independent living is designed to support a full, comfortable lifestyle for seniors. At Concordia, your options for independent living include spacious one- or two-bedroom apartments or patio homes. An independent senior living community It provides residents with activities, services and other amenities while freeing them from heavy housekeeping and home maintenance chores.
Assisted living is for those residents who require help with routine activities of daily living (ADLs), but do not need nursing care. Trained staff is available 24-hours a day to help with meals, dressing, monitoring medications and personal hygiene. There is a licensed nurse on staff at all times.
Concordia offers a separate, secure memory care residence on-site, interconnected with the Health Center. Specially trained staff is available 24-hours a day to assist with personal care, social activities, dining, and monitoring medications. Residents have full access to a secured outdoor patio and garden area to enjoy in addition to their private living suite.
Short-term nursing and re-habilitation care centers provide housing, medical and rehabilitative therapies to people of all ages recovering from illness, injury or surgery in order to successfully transition from an acute care hospital stay to home. Rehabilitative therapies provided at short-term rehab centers may include physical, occupational, speech and respiratory therapy. Short-term rehabilitation may also be referred to as “Skilled nursing care”. Care and therapies provided in this setting may be covered be covered by Medicare.
Long-term nursing care provides residents with accommodations, supervision, and non-acute medical and nursing, and palliative care for those adults who have chronic or end of life conditions. Long-term nursing care centers provide accommodations, nursing care, social and end of life care to those whose needs can no longer be safely met in the home. Medicare does not typically cover costs for long-term nursing care. The Medicaid program provides medical benefits to low-income people who have no or inadequate private insurance or funds for long-term care.
There are two types of fees associated with a CCRC: Entrance Deposits and Monthly Service Fees. The Entrance Deposit is the one-time, upfront investment for the residence and the contract that guarantees residents’ lifetime access to the continuum of care for the balance of their lives. The entrance deposit is based on the health benefit plan, size of residence, number of occupants and the refund plan selected.
Monthly service fees are based on the health benefit plan, size of the residence and number of occupants. These fees provide each resident with certain services and amenities such as meals, transportation, housekeeping, maintenance, utilities, and linen service.
The Residency Agreement can help protect your assets with a choice of refund options including: 90% refundable, 50% refundable and fully declining refund entrance fee plans. The Residency Agreement offers lifetime residency of the apartment or patio home of your choice as well as the security and peace of mind knowing you have guaranteed access to the complete on-site continuum of services and care at Concordia’s on-site Health Center. Residents pay a monthly fee to cover the cost of services.

An entrance fee retirement community is an investment in Life Care, with a continuum of care guaranteed, from assisted living, to rehabilitation, to skilled nursing, memory care and long-term care, if the need arises. Unlike a monthly rental community, where health care is not guaranteed, residents of a Life Care entrance fee community don’t have to worry about the financial risk of rising health care costs. Our on-site health services include traditional assisted living, skilled nursing care, rehabilitation, long term care and memory care. Long and short term health care is paid for on a “fee-for-service” basis.
Traditional Assisted Living: Concordia’s assisted living lifestyles provide residents with the help they need with their activities of daily living in a comfortable, residential environment. It is designed to maintain a resident’s optimal level of independence and autonomy.
Nursing Care: Concordia’s Nursing and Rehab Center operates a licensed nursing care program, where qualified, licensed and certified professionals provide 24-hour care.
Skilled Nursing Facility (SNF): Concordia operates a Medicare certified Skilled Nursing Facility that provides the highest quality of nursing and rehabilitative care. Our professional staff is dedicated to helping residents attain the highest level of independence by meeting their individual needs.
In assisted living, seniors maintain control and independence while receiving assistance with daily activities such as bathing and dressing. Around-the-clock nursing staff is usually not available. Ask for a tour and visit the center during off hours. Sample food, check out the activities schedule, visit with residents and ask about talking to family members. Ask the administrative team about its current survey history. Most locations require private payment, so ask about cost and consider whether this works with your financial situation.
Concordia was founded through an association of ten Lutheran Churches in the Oklahoma District of the LCMS. Concordia has additional, lockable indoor storage space for your seldom used possessions and/or seasonal items within the building.
One of the benefits of Concordia is our extensive offering of educational and recreational activities. We offer a regular schedule of lectures, music, art, movies, entertainment, health and wellness programs, trips in our own bus, gardening, bridge groups, and many other programs. A few examples of the range of programs we offer are Tai Chi, fitness programs, transportation to local churches, social hour, travelogues, Wii bowling and spiritual/religious services for all faiths. Concordia has an fitness center, community meeting rooms, living room, creative arts room, library, beauty salon/barber, and concierge.
Guests of any age are always welcome at Concordia because it is your home.
Your home at Concordia is yours to do with as you please, including entertaining overnight guests in your apartment or patio home. We also offer on-site guest suites at a reasonable cost.
Concordia has lounges for reading, socializing or throwing a private dinner party or gathering. Our Lifestyles Director conducts many classes, activities and special interest groups which you may wish to join. In addition, your own group is welcome to meet at Concordia in any of the available common areas.
Concordia has partnered with Mercy Hospital to staff an on-site wellness clinic with medical director, lab services, visiting physicians, and pharmacy services. If you have a minor illness, such as the flu, you can remain in your apartment or patio home. Tray service is available from one of our dining venues so that you can dine in your home. If a temporary stay or permanent transfer is needed in the health center for a more serious illness or recovery from surgery, you are guaranteed priority access to our on-site nursing and rehabilitation center.
You can purchase additional meals for yourself or guests in the dining room.
Your patio home or apartment at Concordia has a fully equipped kitchen so that you may prepare your own meals. You may also purchase additional meals in the dining room, order a sandwich of the day, or dine out in one of the many area restaurants.
Should your plans change, just notify us in writing to have your reservation deposit returned. There is no risk since reservation deposits are fully refundable.
When you are settled in a house you love, it is difficult to make a big move to a retirement community. However, your good health is your best assurance for entrance. If you wait until you need assistance, it may be too late to quality for residency. Making a Reservation Agreement well before you think you need to move is strongly recommended. Our Wait List option is for individuals who are planning for their future and would like to have a priority for their choice of residence.
